Get Parent to fill and sign on Page one of Pink Form.
You have to personally submit the Pink Form to your nearest
Community Centre (CC) BY HAND. between 25 Feb to 4 March 2011
Do not send by Post.
Successful Applicant will received a notification Letter from the CC.
You are required to go to the CC within 2 months from the date stated
in the notification Letter, to collect the Bursary Cheque.
